■ 2023年度 インフルエンザワクチン接種について ■/2023 influenza vaccination

当院では10月2日（月）より「インフルエンザワクチン接種」を開始致します。
以下、「注意事項」を必ずお読みいただいた上で、直接ご来院ください。
➤ 予約制ではございませんので、事前連絡は必要ありません。

＊防腐剤を含まない（チメロサールフリー）ワクチンの取扱いはございません。
＊16歳以上の方が対象となります。

【 接種期間 】
　2023年10月2日（月）～ 2024年1月31日（水）
　ワクチンの在庫がなくなり次第、終了致します。

【 受付時間 】
　月・水・金曜日　15:00～17:00
　　　　　「上記以外の曜日・時間帯」での受入れは出来兼ねます。

【 料金 】
　5,500円（税込）
　港区にお住まいの65歳以上の方は無料です。

【 当院で接種可能な方 】
　16歳　高校生以上
　＊インターナショナルスクールの場合：高校２年生以上

【 お持ちいただくもの 】
　➤ 65歳以上で23区内にお住いの方・港区助成対象の高校生の方
　　　□（お持ちの方のみ）診察券・保険証
　　　□ 居住区より送付された予診票
　　　　必要事項を記載した上でご持参下さい。
　　　□ ご本人確認証
　　　　住所確認が出来る「運転免許証・マイナンバーカード」等。
　➤ 65歳以下の方
　　　□（お持ちの方のみ）診察券・保険証

【 注意事項 】
　・他のワクチン接種をした方は、各ワクチンで定められている接種間隔を守りご来院ください。
　・「37.5℃以上」および「感冒症状等の体調不良」がある方は接種できません。
　　来院後、医師の問診により、接種不可となる場合もあります。
　・当院では「防腐剤の入らないインフルエンザワクチン」の用意はございません。
　　妊娠している方は通院されている産科にご確認ください。

【 所要時間 】
　来院後、「インフルエンザ問診票」記載・医師問診の後にワクチン接種のながれとなり、所要時間は「30分程度」となります。
　ただし、現場状況により待ち時間が発生する場合もございます。
　お時間に余裕をもってご来院いただきますようお願い致します。

We will start influenza vaccination on Monday October 2, 2023.
Please follow the precautions as below when visiting our clinic and come directly to the outpatient reception. Vaccines will be given in the order of visits.
　➤ We do not take reservations.

＊Thimerosal-free vaccines are not available at our clinic.
＊Inoculation age is 16 years old and over.

【Duration of inoculation】
　October 2, 2023 to January 31, 2024
　＊Vaccination will be terminated as soon as the influenza vaccine is out of stock.

【Reception hours】 Monday, Wednesday, and Friday
　3: 00 p.m. – 5: 00.p.m.　
　*No acceptable for any other days or times other than listed above.

【Inoculation cost】 5,500 JPY (incl tax)
　Free for those over 65 years old who live in Minato Ward, Tokyo.
【Inoculation age】 Over high school students who is 16 years old and over.
　*International school: Over equivalent to second grade of Japanese high school students.
【What you need to bring】
　➤Over 65 years old and residents in Tokyo 23 wards / High school students eligible for Minato-ku subsidies.
　　□Clinic card (only those who have)/ Japanese Health Insurance card
　　□Pre-vaccination screening questionnaire from residence district
　　　＊Please filled in before your visit.
　　□ID card
　　　＊Such as a Driver’s license or insurance card that can confirm your residential address.
　➤Other than the above
　　□Clinic card (only those who have)/ Japanese Health Insurance card

【Precautions at the time of visit】
　・Be sure to follow each appropriate interval for other vaccination if you had or will have one.
　・If you are not feeling well with common cold-like symptoms or body temperature of 37.5℃ or higher, please change the inoculation date.
　　You may not able to have a vaccination depend on pre-interview by doctor.
　・Thimerosal-free vaccines are not available at our clinic.
　　If you are pregnant, please ask your obstetrician.
【The time required】
　・Influenza vaccination flow: At the reception, fill in the “Covid-19 Questionnaire” and after a pre-interview by doctor in the consultation room, you have a flu shot.
　・It takes approximately 30 minutes, however waiting time may occur depending on the situation.
