Commitment to Protection of Patients' Privacy

To protect patients' personal information, Roppongi Hills Clinic (Care Regency Corporation) has designed its privacy policy covering the Act for Protection of Personal Data, which is planned to be fully enforced on April 1, 2005.

We will handle our patients' personal information safely and appropriately according to this privacy policy.

Privacy Policy for Handling Patient Information

This privacy policy defines fundamental rules for handling personal information about those who use our medical and healthcare services (hereafter referred to as "patients"). The personal information here means personal data as defined in Chapter 2, Section 1 of the Act for Protection of Personal Data (hereafter referred to as "law"), specifically, information about existing individuals, which includes such elements as name, birth date and other data or descriptions that enable identification of specific individuals directly or in combination with other information. "Government ordinance" here means the "Enforcement Order of the Act for Protection of Personal Data."

  1. 1 Purpose of Personal Information Use

    Roppongi Hills Clinic uses its patients' personal information for the following purposes (see Notes 1, 2 and 3).

    1. (1) Use within the Clinic

      1. a. Implementation of medical and health care services
      2. b. Medical billing

        Insurances (medical, elderly care, workers accident) and public expenses

      3. c. Management of the Clinic

        Basic data for accounting; internal reports, maintenance and improvement of medical services; support to medical training provided within the Clinic; case research done within the Clinic for better medical practices; and other administrative affairs

    2. (2) Use by contracted organizations

      1. a. Sample examination by contracted laboratories
      2. b. Clerical outsourcing to contracted professionals of insurances (medical, elderly care, workers accident) and publicly held medical services
    3. (3) Disclosure to the outside

      1. a. Cooperation with other hospitals, clinics, maternity centers, pharmacies, home health care agencies, elderly care service businesses, etc.
      2. b. Response to inquiry from other medical institutions
      3. c. Consultation with external specialists about medical treatment of the Clinic's patients
      4. d. Illness explanation to patient's family
      5. e. Submission of medical bills to claim examiners; response to inquiry from claim examiners or insurers; and making claims for payment of medical/elderly care/workers accident insurances
      6. f. Submission of medical bills to administrative agencies in charge of publicly held medical services; response to inquiry from such agencies; and billing for expenses of publicly held medical services
      7. g. Submission of corporate/company medical checkup results to the client corporations/companies
      8. h. Consultation with or report to medical organizations and insurance companies about medical professional liability insurance
      9. i. Disclosure to external auditing organizations
    • Note 1: The Clinic may make changes to these purposes only when such changes are reasonably relevant with the original (Chap. 15 Sect. 2 of the law). Change to the purposes will be notified of individually or in public (Chap. 18 Sect. 3 and 4 of the law).
    • Note 2: Personal information will not be used for any purpose not provided here without prior consent of the relevant patient unless it is legally permitted (Chap. 16 Sect. 3 of the law).
    • Note 3: When a patient or patient's representative authorized by government ordinance requests notification of the purpose of personal information use, the Clinic will immediately respond to the request unless the case is legally defined as an exception (proviso clause in Chap. 24 Sect. 2 of the law).
    • Note 4: If any disagreeable article is found in "(3)@Disclosure of information to the outside", please notify the Clinic's information desk below, otherwise it will be regarded to be fully agreed. The notification of disagreement can be withdrawn or changed at anytime.
  2. 2 Personal Information Management

    The Clinic maintains patients' personal information accurate and updated, with security efforts to prevent leak, loss, destruction or alteration.

    When entrusting personal information to external organizations to serve the purposes provided here, such organizations are properly under the Clinic's supervision for security of information.

  3. 3 Disclosure of Personal Information to the Third Party

    Disclosure of patients' personal information to the third party is limited to serving the purposes provided in "(3)@Disclosure of information to the outside" (see Note 5). Minimum required items are disclosed in the form of written, filmed, electronic, magnetic or orally-communicated data.

    On personal request from a patient, the Clinic will discontinue the disclosure of personal information that can enable identification of the patient.

    When information disclosure to the third party is necessary otherwise than described here, the Clinic must acquire prior consent of the relevant patient (see Note 6).

    • Note 5: Disclosure to the third party does not include the case where personal information is entrusted to contracted organizations to serve the purposes provided here (Chap. 23 Sect. 4-1 of the law).
      The case where disclosure to the third party is legally permitted (Chap. 23 Sect. 1-1 to 1-4 of the law) is excluded.
  4. 4 Disclosure of Personal Information

    When a patient or patient's representative authorized by government ordinance requests disclosure of personal information, the Clinic will respond properly in compliance with the law.

  5. 5 Correction of Personal Information

    When a patient or patient's representative authorized by government ordinance requests that personal information be corrected, added, deleted, suspended, eliminated or concealed from the third party, the Clinic will immediately start examining the request and respond properly in compliance with the law.
